This letter is for undergraduate students who will be either new or transferring into Wheaton College in the Fall 2018.

Dear New Student,

Welcome and congratulations from Student Health Services (SHS) on your acceptance to Wheaton College. Our staff is dedicated to providing you with quality health care so that you will be healthy and well during your time at Wheaton College and after graduation.

This letter is for all incoming students and contains some very important information, requirements, deadlines and services. If you should have further questions after reading this letter or about the requirements, do not hesitate to call SHS at 630-752-5072 or email Student.Health.Services@wheaton.edu. The SHS website contains the entrance requirements and forms.

SHS may send out other notifications about these requirements and deadlines. These notifications may come to the address that you, the student, presented to Admissions when you first applied and via your Wheaton e-mail account. Please read these notifications carefully so that you can avoid unnecessary late fees and registration holds.

Health Entrance Requirements: Completed Forms for Medical History, Physical Exam, Immunizations and Tuberculosis Screening

In order to establish your confidential personal health file; to identify any special health needs; and to comply with Illinois State post-secondary laws, please read the following closely. Each student is required to complete all of medical requirements. Please take the forms to your physician or clinician (MD, DO, NP, PA only) and complete the following: the medical history forms (to be filled out by student in full), physical examination (sports physical unacceptable) (may include blood tests per physician discretion) , tuberculosis screening and immunization certificate. For more specific requirements, please refer to the Entrance Requirements tab. The State of Illinois and Wheaton College require certain immunizations with valid dates and a physician's stamp before your college registration is complete. The completed forms are due to Student Health Services before 11:59pm CST on July 5, 2018. If you are not able to comply with the deadline, you must contact in writing by June 15, 2018 by 4pm CST and your letter will be reviewed by one of the clinicians. Provisions may or may not be provided. Students not in compliance with the medical history form, tuberculosis screening, immunizations and the physical examination requirements by this deadline will be assessed a non-refundable $100.00 late processing fee and placed on registration hold. The registration hold will only be released after the requirements have been fully met. Transfer students may provide a copy of their physical exam, immunization records and tuberculosis screening from their previous college for a SHS clinician review. Medical forms and records can be scanned and emailed to , faxed to 630.752.5575 or mailed to Student Health Services, 501 College Ave., Wheaton IL. 60187. Do not send these forms to the Admissions Office or the Athletic Department.

Communication: Per the student handbook, all College communication is given to the student by utilizing their my.wheaton email account. SHS cannot track Gmail or Yahoo email accounts.

Wheaton College is required by the States of Illinois and Wisconsin to provide incoming and transfer students with disease and prevention notification for meningitis and hepatitis B.

Student Health Services: Nurses, Physicians, Nurse Practitioners, Massage Therapist

SHS is a fully equipped primary health care medical office conveniently located in North Harrison Hall. ALL students and their spouses are able to utilize these services regardless of their health insurance coverage. Students have access to visits with nurses on a walk-in basis. A fee is charged for physician and nurse practitioner visits, lab tests, medicines and some procedures. These charges are transferred to the student account for payment. An explanation of charges form may be sent to the patient‘s college mailbox to be submitted to the insurance company for possible reimbursement. SHS does not bill health insurance companies directly.

SHS utilizes an electronic medical records software program. Students can have access to portions of their chart, via MyChart, by obtaining an individualized passcode.

SHS is open Monday- Thursdays 8:00am-5:00pm and Friday 8:00am-4:00pm during the academic year. Other available services are listed on the SHS website.  

Student Health Insurance

Currently Wheaton College does not offer an Affordable Care Act (ACA) compliant student health insurance plan for students who are U.S. citizens. Please view the Student Health Insurance website for recommended guidelines and requirements to ensure that students are adequately insured while on campus. Some students may also have specific insurance requirements based on programs and/or visa status. All students with an F1 Visa are required to be enrolled in an accident and illness plan that is effective August 1, 2017 through July 31, 2018.* Enrollment is automatic so no action is required by the student. *If entering mid-year, please email health.insurance@wheaton.edu to confirm enrollment.
