This letter is for graduate students who will be entering the Graduate program in the Fall 2018 on the Wheaton, Illinois campus. If you are in a graduate program that never comes on campus, you do not need to comply with requirements.

Welcome and congratulations from Student Health Services (SHS) on your acceptance to Wheaton College.
This letter contains some very important information, requirements, and deadlines and has been sent to you as a personal notice/reminder. Please read the following information carefully so that there is no delay in your registration. Be sure you can access your my.wheaton email account as important notifications are sent there as this is the official communicator for all Wheaton College departments. If you should have further questions after reading this letter about the requirements, do not hesitate to call SHS at 630.752.5072 or email at Student Health Services. The SHS website also contains the entrance requirements, forms and services.
In order to establish your confidential personal health file, to identify any special health needs, and to comply with Illinois State post-secondary laws, each student is required to complete all or a portion of the medical requirements. Please refer to entrance medical form included with this letter for the following requirements:
If you are taking 6 or more credits per semester or are degree-seeking- the medical history form (to be filled out by student), physical examination within the prior year to the deadline, tuberculosis screening questionnaire, and immunization certificate/official copy of immunization records.
If you are taking 5 or less credits per semester you will need to complete the medical history page and the tuberculosis screening questionnaire. There may be additional requirements based on your program. Please contact Student Health Services for specific requirements if you are a graduate student in a modular program, EVAM, TESOL, or in a cohort program.
The State of Illinois and Wheaton College require certain immunizations with valid dates and a physician's stamp before your college registration is complete. The forms are due to Student Health Services before 11:59pm CST on July 5, 2018. If you are not able to comply with the deadline, you must contact Student Health Services in writing by June 15, 2018 by 4pm CST and your letter will be reviewed by one of the clinicians. Provisions may or may not be provided. Students not in compliance with the medical history form, tuberculosis screening, immunizations and the physical examination requirements by this deadline will be assessed a non-refundable $100.00 late processing fee and placed on registration hold. The registration hold will only be released after the requirements have been met. Please let SHS know how we can assist you.
