Attest Vaccination Status

Spring 2022

By January 1, 2022 all students need to attest to their COVID-19 vaccination status and upload proof of vaccination, if applicable, using this form. Log-in using your NetID and password to gain access to the form.

  • Returning Students: If you have become vaccinated since the end of the fall 2021 semester, please upload your COVID vaccine record to the attestation form. In addition, please notify Student Health Services so they are aware that your vaccination status can be updated.
  • New Students: All unvaccinated undergraduate students will receive their test packet in their living space upon arrival. All students that live off-campus and graduate students will pick up their test kits, along with their ID card, at Public Safety.

We expect all students, regardless of vaccination status, to take a PCR or rapid antigen COVID-19 test prior to returning for the spring semester. If you have a positive test or are experiencing any COVID-19 symptoms, please delay your arrival to campus and notify Student Health Services to discuss a plan for your return. 

Any new or returning student who is currently part of the weekly testing plan should submit his or her first saliva sample on the first day of arrival to campus and then resume the regular testing schedule as followed during the fall semester. Please retrieve your new testing supplies from CPO (undergraduate students) or Mary Yeboah (graduate students) before leaving for the Christmas break.
