Dear Student,
Welcome and congratulations from Student Health Services (SHS) on your acceptance to Wheaton College.
This letter contains some very important information, requirements, and deadlines. I would encourage you to please read the following information carefully so that there is no delay in your registration. Please be sure you can access your my.wheaton email account as important notifications will be sent there as this is the official communicator for all Wheaton College departments. If you should have further questions after reading this letter about the requirements, do not hesitate to call or write SHS at 630-752-5072 or Health.Services@wheaton.edu. The SHS website also contains the entrance requirements, forms and services.
In order to establish your confidential personal health file, to identify any special health needs, and to comply with Illinois State post-secondary laws, please read the following closely. Each student is required to complete all or a portion of the medical requirements.
□ If you are taking 6 or more credits per semester or are degree-seeking- the medical history form (to be filled out by student), physical examination (blood tests may be required by your physician) within the prior year to the deadline, tuberculosis screening and immunization certificate.
□ If you are taking less than 6 credits per semester you will only need to complete the medical history page of the medical form and the tuberculosis screening.
The State of Illinois and Wheaton College require certain immunizations with valid dates and a physician's stamp before your college registration is complete. The forms are due to Student Health Services by May 16th for first Summer Session and by June 14th for second Summer Session. Please contact Student Health Services if you are an international student and will not be able to complete these requirements by this date. Students not in compliance with the medical history form, tuberculosis screening, immunizations and the physical examination requirements by this deadline will be assessed a non-refundable $100.00 late processing fee and placed on registration hold. The registration hold will only be released after the requirements have been met.
If you are a non-traditional graduate student (e.g., modular, certificate programs), entrance requirements may vary depending on your program.
Institute for Prison Ministries: If you are taking less than 6 credits per semester you will only need to complete the medical history form and the tuberculosis screening. If you are taking more than 6 credits per semester, you will also need proof of immunity for mumps, measles, rubella, tetanus and diphtheria. Evangelism and Leadership: If you are taking less than 6 credits per semester you will only need to complete the medical history form and the tuberculosis screening. If you are taking more than 6 credits per semester, you will also need proof of immunity for mumps, measles, rubella, tetanus and diphtheria. Eastern European Tutorial: if you are taking 6 or more credits per semester or degree-seeking: the medical history form (to be filled out by student) physical examination, tuberculosis screening and immunization certificate. If you are taking less than 6 credits per semester you will only need to complete the medical history form and the tuberculosis screening. SHS offers services and vaccinations for a low cost if you need assistance in completing these requirements.
Upon your arrival to campus, feel free to come meet our staff and tour the facility. The Student Health Services is located in the center of the campus for your convenience. We are here to help make your experience at Wheaton College a healthy one.
