Due to the fact that the ITC is a specialized clinic, non-students utilizing this services will be asked to present the following primary care records 48-hours before their travel consult:
- Documentation of vaccinations or blood work to show immunitites
- A current physical if > 40 years old within the last year
- A current physical if = 40 years old within the last 3 years
All non-student clients will need to pay for their travel consult, travel supplies, and any vaccines administered AT THE TIME OF SERVICE. Cash or check only, please.
If the SHS Travel Physician should need further clarification of your medical needs or history, she may contact your Primary Care Provider.
All patient specific medical information sent to SHS or completed at SHS complies with all federal and state privacy and security laws.
