Student Health Insurance Claims
If you are on the Student Health Insurance plan, you must submit a claim form to the insurance company with any new illness or injury. You may print a claim form out (see bottom of page for pdf) and mail it in for any medical services you have completed. For more information regarding claims, please go to www.Klais.com.

Athletic Sports Insurance
If you are a varsity student-athlete injured during the official season of play, you must fill out the appropriate claim form. Please contact the Athletic Training Office at 630-752-5738 for further assistance. The claim form and letter are listed below for your convenience.
